Required Documents

  1. Proof certifying that there is an arbitration agreement between the two parties.

  2. Include a summary of the incident, detailing the dispute, type and amount of securities, purchase/sale dates and values, and their classification by market listing.

  3. Provide a clear claim summary, including claim, compensation, and interest values. If claiming shares in kind, estimate value using the prior business day's closing price before arbitration filing

  4. Attested power of attorney if the arbitration request is being filed by someone else other than the arbitration applicant

  5. Any other documents supporting the request.
