Required Documents

  1. The application form, (FORM 20300), accompanied with the following documents: Copy of the non-objection letter issued by the current legal representative.

  2. Copy of the letter giving authorization for the new legal representative. The letter must be approved by the fund manager, the management company, or the investment manager.

  3. Form 10004 giving authorization for the new legal representative to sign and handle the applications submitted to the CMA.
